The aim of this (15)O-labelled H(2)O bolus positron emission tomography (PET) study was to analyse the hemispheric dominance of the vestibular cortical system. Therefore, the differential effects of caloric vestibular stimulation (right or left ear irrigation with warm water at 44 degrees C) on cortical and subcortical activation were studied in 12 right-handed and 12 left-handed healthy volunteers. This study showed the posturographic parameters that were useful for assessing the functional ability to maintain balance in our sample of vestibular patients. Material and methods Of a total of 89 patients, 59 were healthy subjects and 30 had a peripheral vestibular disorder. The subjects were studied using the posturographic NedSVE/IBV system, combining static (Romberg) and dynamic (stability limits and rhythmic weight shifts) tests. Effect of water immersion on post-rotatory and caloric nystagmus.

Caloric nystagmus and post-rotatory nystagmus were recorded with and without head-out water immersion. As water immersion reduces body weight by about 90% owing to buoyancy, it decreases somatosensory inputs. Thus water immersion can be used to simulate a weightless environment. Caloric nystagmus was enhanced significantly by water immersion; however, post-rotatory nystagmus was not. These findings demonstrate that caloric nystagmus is easily affected by water immersion which decreases somatosensory inputs, but that post-rotatory nystagmus is not.

The findings demonstrate that an enduring unilateral utricular dysfunction, possibly together with canal hypofunction, can occur after labyrinthine disease or injury. They also suggest that unilateral, isolated utricular dysfunction - or utricle paresis - can occur, representing a novel entity in the differential diagnosis of peripheral vestibular function. Control experiments were carried out on the same subjects pre- and postflight.
